Breast Implant Illness: Symptoms, Causes and What You Can Do in 2026

More than 400,000 people in the United States get breast implants every year. Most of them are never told that a growing number of women will develop a collection of symptoms so widespread and debilitating that it has its own name: breast implant illness.

BII is not yet a formal medical diagnosis. There is no blood test for it, no imaging scan that confirms it, and no billing code your insurance company recognizes. But the symptoms are real. The research is growing. And the FDA is paying closer attention than ever before.

What Is Breast Implant Illness?

Breast implant illness (BII) describes a range of systemic symptoms that develop in people after receiving breast implants. The condition goes by several names in medical literature, including autoimmune/inflammatory syndrome induced by adjuvants (ASIA) and silicone implant illness.

The symptoms occur regardless of implant type. Silicone gel-filled, saline-filled, smooth surface, textured surface, round, or teardrop-shaped. It does not matter. Women with every type of implant have reported BII symptoms.

Researchers suspect the condition may stem from an autoimmune or inflammatory reaction to components of the implant, particularly silicone. Bacterial biofilm formation on the implant surface and the body's ongoing immune response to a foreign object are also being studied as contributing factors.

BII can appear immediately after surgery. Or it can take years. Some women live with implants for a decade before symptoms begin.

The Most Common Symptoms

A population-based study published in the Annals of Plastic Surgery surveyed 182 women who identified with breast implant illness. The numbers were striking:

  • Brain fog: 95% of respondents
  • Fatigue: 92%
  • Joint pain: 80%
  • Hair loss: 74%

Those are the top four. But the full list is much longer. Cleveland Clinic reports that people with BII have described more than 50 different symptoms spanning multiple body systems.

Musculoskeletal symptoms include joint pain, muscle pain, and muscle weakness.

Cognitive symptoms include memory loss, trouble concentrating, and the persistent mental cloudiness that the BII community calls "brain fog." Women describe it as thinking through mud. Forgetting words mid-sentence. Losing the ability to focus on tasks that used to be automatic.

Systemic symptoms include chronic fatigue, autoimmune condition symptoms or new autoimmune diagnoses, dry eyes, skin rashes, and chronic pain that moves around the body without explanation.

Neurological symptoms documented in case studies include anxiety, depression, tinnitus, dizziness, vision changes, and heart palpitations.

A scientific review published in Taylor & Francis found that many BII patients meet the clinical criteria for Chronic Fatigue Syndrome (CFS/ME). Unlike ordinary tiredness, these patients wake up severely symptomatic, and their fatigue is not relieved by rest. Physical or mental exertion can trigger symptom flares lasting days or weeks.

What makes BII so difficult to pin down is that nearly every symptom on the list overlaps with other conditions. Lupus, rheumatoid arthritis, Lyme disease, fibromyalgia, hypothyroidism, and menopause can all produce similar complaints. That overlap is exactly why so many women spend years bouncing between specialists without answers.

Who Is at Risk?

Anyone with breast implants. There is no known genetic marker, body type, or pre-existing condition that predicts who will develop BII and who won't.

The condition affects women with implants placed for cosmetic reasons and women with implants placed for reconstruction after mastectomy. It affects women with new implants and women who have had them for 20 years.

Breastcancer.org confirms that BII symptoms can develop whether implants have ruptured or remained intact. The implants do not need to be broken to cause problems.

There is some research suggesting that women with MTHFR genetic variants, which affect the body's methylation and detoxification pathways, may be more susceptible. More than half of respondents in BII community polls reported having MTHFR variants. This is an area where more formal research is needed.

How BII Is Diagnosed

There is no standard diagnostic test for breast implant illness. Diagnosis happens by exclusion.

Your doctor will review your symptoms and medical history. Depending on what you describe, they may order bloodwork to rule out autoimmune conditions, thyroid disorders, Lyme disease, or other conditions with overlapping symptoms.

If everything comes back normal and you have breast implants, BII becomes a strong possibility.

This is one of the most frustrating parts of the experience. Women frequently describe going through years of testing, seeing multiple specialists, and receiving no clear diagnosis before they connect their symptoms to their implants. A plastic surgeon in Austin, Texas who specializes in BII treatment told breastcancer.org that most of his patients find him after exhausting all other medical options.

The lack of a formal diagnostic code also means that many doctors are not yet familiar with BII. If your provider dismisses your concerns, seek a second opinion from a surgeon who has experience with implant-related illness.

What the Research Says in 2026

The science around BII is catching up to what patients have been reporting for years.

A systematic review and meta-analysis published in Aesthetic Plastic Surgery in August 2025 analyzed the existing body of research on BII symptoms, outcomes after explantation, and potential causes. The review confirmed that the condition is characterized by a broad spectrum of systemic symptoms and that explantation (implant removal) leads to symptom improvement in the majority of patients.

The Aesthetic Society has established a BII task force conducting a long-term study tracking new breast implant patients over more than 10 years. This is the kind of large-scale, prospective research that the medical community has been lacking.

The FDA updated its breast implant safety data in February 2025, confirming ongoing Medical Device Reports of systemic symptoms including fatigue, brain fog, joint pain, and autoimmune-like symptoms linked to implants. The agency now requires a black box warning on all breast implant labeling and mandates that surgeons review a patient decision checklist before implant surgery.

We are past the point of debating whether BII is real. The question now is understanding the mechanisms behind it and determining the best ways to support women who are affected.

Treatment: What Are Your Options?

The most common treatment for BII is explantation, the surgical removal of the implants and, in many cases, the scar tissue (capsule) surrounding them. When the implant and capsule are removed together, it is called an en bloc capsulectomy.

The population-based study found that 97% of women who had their implants removed reported relief from their symptoms. Of those, 23% experienced complete resolution and 74% experienced partial improvement. Relief often begins within the first 30 days after surgery.

But explantation is a major decision. It involves surgery, recovery time, changes to appearance, and cost that insurance may not cover. Not every woman is ready or able to explant.

For women who choose to keep their implants, or who are waiting for surgery, or who want additional support during recovery, nutritional supplementation can play a role.
